The KRG-U.S. office is able to help provide documentation forms for individuals to create powers of attorney and related legal matters. The PDF forms below will cover the majority of needs.

Once you find the forms that are appropriate, please complete and sign THREE copies of the forms and have those forms notorized by a public notary in the United States or Canada.

Please include three colored passport size photos of the applicant with the forms. Also include copies of the applicant’s identity card and Iraqi ID card (jensya).

Enclose money order or personal check made payable to K.R.G. as the application fee.

Please ensure that you include contact info full name, address, phone number and email if available, and mail all materials to KRG 1532 16Th Street, Washington D.C 20036 USA.
